*Overview* The Residential Program Coordinator (PC) oversees the operations for multiple residential program sites.
The PC provides leadership to supervisors and staff working in the assigned programs, coordinates relations and care with clients, case managers, guardians and families, facilitates and administers oversight in all client and program business.

00_ Base Starting Pay: * $50,000-$55,000/yr Location: * Grand Rapids, MN * Chisholm, MN Time type: * Full-Time *Responsibilities* * Supports the Mission, Vision, and Guiding Principles of NHS Northstar.
* Provides direct oversight of supervisory personnel; develops procedures to regularly giving and receiving formal feedback on performance matters; develops employee development strategies to proactively manage forecasted staffing needs; coaches, counsels, and disciplines staff (including mid-management staff) who fail to meet performance expectations.
* Attends and leads team meetings when appropriate to address important issues, maintains a visible presence and supports front-line supervisory staff.
* Coaches Residential Supervisors in implementing Coordinated Services Support Plans and other care plans.
Follows up regularly to monitor their progress in implementation.
* Conducts regular inspections of client sites; reviews employee and client documentation; provides feedback to staff as appropriate.
* Evaluates the satisfaction of persons served by the program, the person’s legal representative, if any, and the case manager, with the service delivery and progress toward accomplishing outcomes; ensuring each person’s rights as identified in Minn.

* Manages the new-client intake and discharge process, ensuring a smooth transition for the client.
* Assists clients in identifying personal outcomes and individual preferences; writes program documents incorporating the identified outcomes and preferences; writes the documents according to agency policy/procedure and Minnesota Department of Human Services rules/regulations within designated time frames; ensures efforts are made to achieve the client’s dreams and desires; provides accurate information to the interdisciplinary team.
* Coordinates effectively with external partners such as case managers, guardians, family members and referral sources to promote client retention.
* Designs and implements policies, procedures, and methods to promote quality care, decrease risk to clients, and improve client and referral sources satisfaction.
* Ensures staff competency requirements are met according to the requirements in Minn.

* Ensures corrective action is taken when ordered by the Commissioner of Human Services and that the terms and conditions of the license and any variances are met.
* Evaluates program efficiency and effectiveness by overseeing the Residential Services Supervisor functions and responsibilities to ensure satisfactory conditions at assigned program sites.
* Oversees the scheduling of direct support to ensure the well-being of the individuals served.
* Maintains and manages site budgets by reviewing reports generated by the Residential Services Supervisors and/or Executive Team to ensure compliance with annual budget.
* Monitors psychotropic medication programs and/or Rule 40 programs as assigned or directed by Health Services and/or Director of Residential Services.
* Ability to be available and flexible in duties, scheduling and work location to support business needs.
* Other duties as assigned.
*Qualifications* *Experience/Education/Training* * High School Diploma * Meet the qualifications of a “Designated Coordinator” as identified in Rule 245D as follows: a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or a minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
